On July 13, 2000, USNS Yukon, a replenishment oiler, and USS Denver (LPD-9), an amphibious transport dock,
collided during a refueling exercise west of Hawaii. Both ships sustained significant damage.
An investigation found that “human error caused this collision,” with USS Denver at fault. No injuries were reported.
