On July 13, 2000, USS Denver, an amphibious transport dock, and USNS Yukon (T-AO-202), a replenishment oiler,
collided during a refueling exercise west of Hawaii. Both ships sustained significant damage.
An investigation found that “human error caused this collision,” with USS Denver at fault. No injuries were reported.
